1. RENEWAL /DAMAGE OF PASSPORT
Requirements

- One recent (within 6 months) white background passport-sized photo.
Applicants must bring along their own one photo. Photo-taking services are strictly not available at The Embassy.
- (It is also required to take the photo at Embassy to be used in MRP Passport)
- Original and 2 copies of Passport (Data page and Observations pages )
- Tax receipt ( Copy & Original )(If have any)
- WP/Spass/EP/PR Card ( Re-Entry Form ) (Original and 2 Copies)
If Applicant is Dependent
- List of Households/Birth Certificate/Marriage Certificate ( Original And 2 Copies )
- Copies and Originals of Passport, IC of your host ( For PR, need to submit with Re-entry Form-7 )
If Applicant is Student
- He/She needs to submit Student Pass(Original And Copy)
- School Recommendation letter
- If an Applicant is a seaman, It needs to provide CDC (Original and Copy) and Certificate of Completion on Basic Safety Training (4-Tickets)
- For the applicants who want to change PJ type passport and holders of PJ type passport, it needs to provide copies and original of WP,S pass,EP&PR card with Re-entry Form-7. For the PR, it may need to show IRAS or Company letter.
- If the applicant is female nationals who married to foreigners and are dependent, she needs to provide the receipt of the air-Ticket fee deposit SGD$500.
If the applicant can not provide original documents, his/her application will be rejected . Please note that Consular Officer reserves the right to request additional documents as necessary
2. LOSS OF PASSPORT
Requirements

- Two recent(within 6 months) white background passport-sized photo
Applicants must bring along their own one photo. Photo-taking services are strictly not available at The Embassy.
- Singapore’s Police lost report
- Copies of Data page and last endorsement page of your lost passport (2 copies )
- WP/ Spass/ EP/Student Pass/ Dependent Pass/ PR Card (Re-Entry Form-7 ) (Original And 2 Copies )
- Myanmar National Identity Registration Card ( 2 Copies )
- Myanmar Households list (front & back) ( 2 Copies)
- Tax receipt ( Copy & Original (If have any))
- If Applicant is Seaman, he will need to provide CDC (Original And Copy) and Certificate of Completion on Basic Safety Training (4-Tickets)

4. SEAMAN TO SHOREJOB

5. CERTIFICATE OF IDENTITY FOR NEWBORN/ LOST PASSPORT
Requirements
- Application Form
- Two recent passport-sized photo (must be taken within 6 months)
Applicants must bring along their own white background passport sized photo. Photo-taking services are strictly not available at The Embassy.
For NEW BORN it is required to bring
- Application for newborn, mother & father (original & copy)
- Newborn birth certificate (original & copy)
- Parent's passport , work permit, myanmar IC if any (original & copy)
- Air ticket
For LOST PASSPORT, It is required to bring
- Application Form
- Two recent assport-sized photo (must be taken within 6 months)
- Police report
- Lost passport copy
- Work permit (original & copy)
- Myanmar Household list (copy of original)
- Myanmar IC (original if any & copy)
- Air Ticket
THE APPLICANT IS REQUIRED TO APPLY FOR CERTIFICATE PRIOR TO ONE WEEK BEFORE DEPARTURE

6. RECOMMENDATION LETTER FOR BEING DEPENDENT

7. AIRTICKET FEE DEPOSIT
Requirements
- Request Form
- Original and a copy of Passport (Photo page and Observations pages)
- Original and copy of IC card or PR Card with Re-Entry Permit(Form-7)
- Original and Copy of Passport and IC of Spouse

9. WITHDRAWAL FOR RETURN AIR TICKET DEPOSIT
This is to inform you that those who have deposited the return air ticket fee can withdraw the deposit under the following conditions:
i. when the applicant has divorced with the foreign spouse
ii. when the applicants has already obtained other Nationality
iii. when the applicants has cancelled stay permit in Singapore and returned to Myanmar for good.
iv. when the foreign husband of the applicants has died
To withdraw the air ticket fee deposit, the required documents will be as follows:
- Request Form
- Personal Letter requesting for refund of Return Airticket Fee
- Original and copy of IC card or PR Card with Re-Entry Permit(Form-7)
- Original and Copy of Passport and IC Copy of Husband
- Original receipt of Return Airticket
- Divorce Certificate(or) Embassy Letter for Renouncement of Citizenship if the applicant has already surrendered Myanmar Citizenship. (or)Death certificate of the husband
Note: (For those who have deposited the Return air ticket fee for over 3 years at the Embassy, they can withdraw the air ticket fee deposit by submitting the following documents:
(1.) Personal Letter requesting for refund of Return Airticket Fee
(2.) IC Copy & Original
(3.) Passport Copy & Original
(4.) Original receipt of Return Air ticket which must be over 3 year periods starting from the date of deposit.
After submitting the applications, the applicants will be informed when she can collect back the air ticket fee deposit.
All the applicants need to submit from 9 am to 1200 pm (Mon to Fri)

10. INVITATION OF MONKS
Requirements
- Request Form
- Copy of Passport of applicant
- Copy of Passport of venerable
- Copy of IC Card(WP/SPass/EP/PR Card (If PR with Re-Entry Permit - Form 7) of applicant
- Buddhist Monk registration ID Card of Venerable monk
- Recommendation letter from the Monastery / Temple where the monk is staying in Myanmar
If the applicant can not provide original documents, his/her application will be rejected . Please note that Consular Officer reserves the right to request additional documents as necessary
11. Notary attestation
Requirements
- Request Form
- For Notary endorsement –
- Notary must be endorsed by the Ministry of Foreign Affairs in Myanmar
- Notary validity must be within (6) months
- Notary must include counter signature of Ministry of Foreign Affairs (MOFA) official and official seal of the MOFA , Myanmar.
If the applicant can not provide original documents, his/her application will be rejected . Please note that Consular Officer reserves the right to request additional documents as necessary
12. Legalisation of Documents
Documents issued in Singapore (including Singapore Government documents, corporate documents, business agreements, others) will be acceptable in Myanmar only if they have consular authentication by Myanmar Embassy in Singapore. Each document that will be used in Myanmar must be individually legalized and certified by the Embassy.
Documents will be certified by the Embassy only after it has been completed the following steps:
Step 1) Notarised by a Notary Public (any law firm offering notarial services) in Singapore
Step 2) Certified by Singapore Academy of Law
Please kindly note that the applicants must bring the individual or Company request letter for legalization.
Submission Time : 9:30 am to 12 pm (noon)
Submission Place: Embassy Information Counter
Collection Time : 3 pm to 4 pm
Legalization fees: S$50 /- for each document, payable by Cash only
Processing Time : 7 Working days
