1. RENEWAL OF PASSPORT

Requirements

  1. Appointment Letter And Application Form (Personal history form) taken by online Appointment Link
  2. One recent photo (must be taken within 6 months)
  3. (It is also required to take the photo at Embassy to be used in MRP Passport)
  4. Original and a copy of Passport (Photo page and Observations pages)
  5. Tax receipt ( Copy & Original )
  6. WP/Spass/EP/PR Card ( Re-Entry Form ) (Original and Copy)
    1. If Applicant is Dependent

    2. List of Households/Birth Certificate/Marriage Certificate ( Original And Copy )
    3. Copies and Originals of Passport, IC of your host ( For PR, need to submit with Re-entry Form-7 )
    1. If Applicant is Student

    2. He/She needs to submit Student Pass(Original And Copy)
  7. If an Applicant is a seaman, It needs to provide CDC (Original and Copy) and Certificate of Completion on Basic Safety Training (4-Tickets)
  8. For the applicants who want to change PJ type passport and holders of PJ type passport, it needs to provide copies and original of WP,S pass,EP&PR card with Re-entry Form-7. For the PR, it may need to show IRAS or Company letter.
  9. If the applicant is female nationals who married to foreigners , she needs to submit the receipt of the air-Ticket fee deposit SGD$500.

If the applicant can not provide original documents, his/her application will be rejected . Please note that Consular Officer reserves the right to request additional documents as necessary

2. LOSS/DAMAGE OF PASSPORT

Requirements

  1. Appointment Letter And Application Form (Personal history form) taken by online Appointment Link
  2. One recent photo (must be taken within 6 months)
  3. Singapore’s Police report letter (loss Passport)
  4. Copies of first page and last endorsement page of your lost passport
  5. WP/ Spass/ EP/ PR Card (Re-Entry Form ) (Original And Copy) (if applicable)
  6. Myanmar National Identity Registration Card (Copy)
  7. Tax receipt ( Copy & Original )
  8. If Applicant is Dependent
    1. List of Households/Birth Certificate/Marriage Certificate (Original And Copy)
    2. Copies and Originals of Passport, IC of your host
  9. If Applicant is Student
    1. Recommendation Letter issued by University/ College/ School concerned or Transcript (Original And Copy)
    2. Student Pass (Original And Copy)

  3. Personal History of New Born (One photo within 6 months)
  4. Personal History of parents (One photo within 6 months)
  5. Marriage Certificate of Parents (Original and Copy)
  6. Myanmar Identity Registration Card of Parents (Copy)
  7. Parents’ passport and IC ( If PR with Re-entry Form-7 ) ( Originals And Copies )
  8. Tax Receipt (Original / Copy)

Requirements

Requirements

  4. List of Households/Birth Certificate/Marriage Certificate
  5. Tax receipt of the person whom the applicant depends on
  6. Original receipt of Return Air Ticket if the applicant is the one who is a dependent to a foreigner

9. AIRTICKET FEE DEPOSIT

Requirements

  4. Original and Copy of Passport and IC of Spouse

10. WITHDRAWAL FOR RETURN AIR TICKET DEPOSIT

        This is to inform you that those who have deposited the return air ticket fee can withdraw the deposit under the following conditions:

        i. when the applicant has divorced with the foreign spouse

        ii. when the applicants has already obtained other Nationality

        iii. when the applicants has cancelled stay permit in Singapore and returned to Myanmar for good.

        iv. when the foreign husband of the applicants has died

To withdraw the air ticket fee deposit, the required documents will be as follows:

  1. Appointment Letter And Application Form taken by online Appointment Link
  2. Personal Letter requesting for refund of Return Airticket Fee
  3. Original and copy of IC card or PR Card with Re-Entry Permit(Form-7)
  4. Original and Copy of Passport and IC Copy of Husband
  5. Original receipt of Return Airticket
  6. Divorce Certificate(or) Embassy Letter for Renouncement of Citizenship if the applicant has already surrendered Myanmar Citizenship. (or)Death certificate of the husband
  7. Note: (For those who have deposited the Return air ticket fee for over 3 years at the Embassy, they can withdraw the air ticket fee deposit by submitting the following documents:
            (1.) Personal Letter requesting for refund of Return Airticket Fee
            (2.) IC Copy & Original
            (3.) Passport Copy & Original
            (4.) Original receipt of Return Air ticket which must be over 3 year periods starting from the date of deposit.
            After submitting the applications, the applicants will be informed when she can collect back the air ticket fee deposit.
    All the applicants need to submit from 9 am to 1230 pm (Mon to Fri)

11. INVITATION OF MONKS

Requirements

  1. Appointment Letter And Application Form taken by online Appointment Link
  2. Copy of Passport of applicant
  3. Copy of Passport of venerable
  4. Copy if IC Card(WP/SPass/EP/PR Card (If PR with Re-Entry Permit – Form 7) of applicant
  5. Buddhist Monk registration ID Card of Venerable monk
  6. Recommendation letter from the Monastery / Temple where the monk is staying in Myanmar
  7. If the applicant can not provide original documents, his/her application will be rejected . Please note that Consular Officer reserves the right to request additional documents as necessary

12. General / Special Power

Requirements

  1. Appointment Letter taken by online Appointment Link
  2. General Power / Special Power Document
  3. Original and Photocopies of Two Witnesses( Ages over 18)
    (Passport & IC/PR Card (If PR needs Re-Entry Permit (Form 7)/EP/Student
    Pass)
  4. Original and Photocopies of (Passport & IC/PR Card (If PR needs Re-Entry Permit (Form 7)/EP/Student Pass) of the applicant
  5. Photocopy of National Identity Card of the nominated person
  6. Personal Application Letter which was addressed to the Ambassador
  7. One working day processing time

Note : At the back side of GP/SP paper, the two witnesses have to fill up their names, passport numbers and addresses.The attorney given person and the two
have to be in the presence in front of the Embassy’s official and sign on the General/Special Power Form. The applicant need to take online appointment and have to come to Embassy at working days.
